Traumatized and afraid to see a dentist having waited an average 8 or more years between visits, Senior Charity Care Foundation (SCCF) wanted to demonstrate to our funders that we are making a difference in the lives of our seniors, including improvement of overall health. SCCF serves residents of long-term care and assisted living facilities and Section 8 HUD funded senior apartments via mobile dental clinics. We collaborated with AARP consultants, Success Measures Evaluation Services from NeighborWorks America, to improve our surveys to include three surveys pre-service, upon completion of the treatment plan, and at 6 months after treatment completion. The pre-survey assesses oral health knowledge, reason for gaps in oral health care, emergency room visits for oral issues, pneumonia history, nutrition and oral health concerns. The post treatment survey ascertains satisfaction of services, fear alleviation, affordability of services, and the follow-up questionnaire is a combination of both previous questionnaires.
