This tele-mentoring session focuses on the role of social determinants of health in educating dialysis patients about the risk of infection with central venous catheters. A roundtable with an interdisciplinary team—including a nephrologist, dialysis nurse, technician and social worker—convenes to discuss communication strategies to reduce infection risk, taking social determinants of health into account.
